Reason To call for Case Management:

 

     Family or caregivers have limited ability to assist with healthcare needs and/or limited knowledge about available resources.

     An increasing number of falls (with or without injury).

     Multiple disease processes or a major diagnosis of stroke, diabetes, paralysis, cancer.

     A decline in a loved one's medical condition or their ability to care for themselves.

     An increasing number of hospitalizations or extended length of stay due to major illness or complications.

     Multiple physicians being utilized and multiple prescriptions being ordered.

     A lack of communication between healthcare providers.

     An increasing number of hospitalizations

     Confusion, concern or lack of information about the current treatment plan.

     Questionable compliance or lack of progress with treatment plans.

     Out of pocket healthcare costs continue to rise or limited health insurance benefits.

     Experimental or investigational care.
