Comprehensive Care Management, LLC (CCM) was founded in order to assist clients and their families through the healthcare decision making processes that can occur with an illness, injury or declining health status. When we are faced with these challenges, our attention is turned towards the issues at hand: what kind of treatment is needed, who will be able to help, what are the physicians talking about and so forth. Today's healthcare systems are a far more complex maze than ever, which has created increased fragmentation in care and confusion amongst providers and patients. The focus of case management is on advocating for the patient, guiding the patient and family, acting as a resource, and coordinating appropriate healthcare services in a timely and cost effective manner.
